How to Get a Refund of the Immigration Health Surcharge (IHS): A Client Guide

This blog aims to demystify the process by explaining when you’re eligible, how to apply for a refund, how long it takes, and who to contact. 

What Is the Immigration Health Surcharge (IHS)? 

The IHS is a fee paid by most visa applicants to access the National Health Service (NHS) in the UK. It’s paid during your visa or immigration application process. As of 2024, the fee is generally: 

  • £1,035 per year for adults 
  • £776 per year for children and students 

When Can You Get a Refund of the IHS? 

You may be eligible for a full or partial refund of your IHS payment in the following situations: 

1. Visa Refused 

If your visa or immigration application is refused or withdrawn, you will usually receive a full refund of the IHS automatically. 

Not refunded: The IHS will not be refunded if your visa is granted but you decide not to travel or use it. 

2. Shortened Visa 

If you are granted a visa for a shorter period than what you applied for, you may be eligible for a partial refund. 

3. Duplicate Payment 

If you accidentally paid the IHS twice (for example, through multiple applications), you may request a refund of the duplicate charge. 

4. Health and Care Worker Exemption 

If you are eligible for the Health and Care Worker visa and have paid the IHS in error, you can apply for a full refund—even if your visa is granted. 

Important: You must not have used any NHS services between paying and the refund being processed unless the service is exempt (e.g., COVID-19 treatment). 

Who Is Not Eligible for an IHS Refund? 

You will not get an IHS refund if: 

  • Your visa is granted and you enter the UK (except in Health and Care Worker exemptions). 
  • You overstay your visa or remain unlawfully. 
  • You leave the UK before your visa expires without cancellation. 

How to Apply for an IHS Refund 

A. Applications Made from the UK or Abroad (Visa Refused or Withdrawn) 

No action is needed. The IHS refund is usually processed automatically within 6 weeks of your visa refusal or withdrawal. 

Refunds are made to the card or bank account used for payment. 

B. Health and Care Worker Refunds 

You must actively apply online for a refund if: 

  • You are eligible under the Health and Care Worker route, and 
  • You paid IHS as part of another visa type.

You’ll need: 

  • IHS reference number 
  • Visa application number 
  • Employer details (NHS or care provider) 
  • Proof of payment 

C. Duplicate Payments or Errors 

If you believe you’ve paid the IHS in error or have been charged twice, contact UKVI or the Home Office directly:

  • Full name 
  • Date of birth

How Long Does It Take to Get a Refund? 

  • Automatic refunds: Usually within 6 weeks from the date of visa refusal or withdrawal. 
  • Manual applications: Typically, 6–12 weeks, depending on complexity and volume. 
  • If you’re still waiting after this period, it’s advisable to contact UKVI with your reference number.
